How to Enroll

Once you are eligible for benefits, you must enroll within 30 days to have the medical and dental options you want. Future changes in your options can be made during the annual Open Enrollment period.

You enroll yourself and your eligible dependents by completing the following forms (found in the Enrollments Materials folder of Your Benefit Package):

  1. TBT Enrollment Form. The process of starting your benefits won't begin until this form is received (see Why Enroll? below).
  2. Medical Option Form to choose your TBT medical option (which must be the same for you and your covered dependents).
  3. HMO Application. If you choose coverage under an HMO, the HMO application is also needed. If you need an HMO packet and application, contact the TBT Plan Administration Office.
  4. Dental Option Form to choose your TBT dental option (which must be the same for you and your covered dependents). If you are a newly hired employee, restrictions may apply (link to Dental Options).
Why Enroll?

There are important reasons why you should not delay sending in your TBT Enrollment Form, Medical Option Form, Dental Option Form and an HMO application (if you choose an HMO).

  • If you do not select a TBT medical option, you will automatically be enrolled in the Indemnity Medical option; however, no claims will be paid until your TBT Enrollment Form and Medical Option Form are received.
  • If you want medical coverage under an HMO, you cannot be enrolled without a TBT Enrollment Form and Medical Option Form plus an HMO application.
  • Your plastic prescription drug card will not be ordered for you until a TBT Enrollment Form is received.
Important
Your plastic prescription card will be mailed to you as close to your eligibility date as possible. If you are eligible for prescription drug benefits, but have not yet received your plastic prescription drug card, you may be reimbursed for covered benefits (link to Prescription Drug benefits).

  • You will have no dental coverage until you send in the Dental Option Form.
  • You won't receive important notices about your benefits because the Plan will not have your mailing address.
  • You won't have named a beneficiary to receive Plan benefits if you die or are seriously injured. You designate your beneficiary on your TBT Enrollment Form.
  • You and your covered dependents may face delays when you need to use your benefits.
  • Health care providers will not be able to verify your coverage.

Enroll Today!

Send in your TBT Enrollment Form (and other required forms) as soon as possible. These forms are in the Enrollment Materials folder in Your Benefit Package. Don't wait until your eligibility date.

Contact the TBT Plan Administration Office for any missing or extra forms which you may need.
