Have you moved or had a dependent change?

Whenever you move, send a Change of Address Form (found in your Benefit Package) to the TBT Plan Administration Office so you'll receive important information about your benefits. If you want to verify your Plan address or other data, contact the Plan Administration Office.

Update Your Records

Have you recently been married, widowed, divorced or had other important changes? Have you added or changed dependents? It is your responsibility to notify the Plan Administration Office about changes that may affect eligibility. Please provide names, Social Security numbers and dates of birth for your dependents.

Keep your address current, so you'll receive up-to-date information about your benefits. Remember, TBT keeps one address for each participant. If your spouse or dependents don't live with you, make sure they know that all TBT mail will be sent to your address.
